Can I collect camp registrations with this tool?
This tool designs a registration form. It does not host live registration, take payments, collect medical details, upload files, or execute signatures. Explore Bookify separately to plan your live workflow.
What is included in my download?
PDF and Word contain a blank form plus purpose and logic review notes. JSON preserves your complete editable design. CSV documents every question and mapping. HTML is a read-only preview with no submission endpoint.
How are family and group forms different?
Shared household or organizer details are collected once. Participant sections can repeat for distinct people. Emergency contacts, representatives, and pickup contacts remain separate roles.
Do I need a name, email, or account to use it?
No. You can build and export without entering any person’s details. Preview text is fictional, and optional browser saving stores the design, never participant answers.
Does this form include a legally approved waiver?
No. The builder helps you separate acknowledgements and optional permissions. Your operator must supply and review any policy, eligibility rule, waiver, or live consent workflow.
